HORIZON MERCY

Horizon Mercy is a healthcare management organization that has been serving the publicly insured in New Jersey since 1994. Horizon Mercy manages access to healthcare services for the publicly insured in New Jersey who enroll in Horizon Healthcare of NJ, Inc.'s managed Medicaid program. Horizon Mercy has developed innovative health outreach and prevention programs. They are committed to strengthening relationships with community-based organizations, churches, grassroots agencies and schools. The SNAP-Ed Program has assisted Horizon Mercy in leveraging resources i.e. nurses, social workers, community educators who have the opportunity to perform outreach by interfacing with SNAP-Ed Program Network members from the state. SNAP-Ed Program sits on Horizon Mercy's Health Education Advisory Committee and they are members of the SNAP-Ed Support Network. SNAP-Ed Program has delivered seminars on Social Marketing to their board. Educators from SNAP-Ed Program county offices have delivered Asthma and Lead seminars as part of the Work First NJ Program. Future projects planned include a "Train the Trainer" in SNAP-Ed Program Nutrition Education Curriculum.
